The Illinois Supportive Living Program (SLP) is an innovative and nationally recognized model that addresses the growing need for affordable, person-centered long-term care. The SLP was developed in response to rising Medicaid costs and the lack of viable housing options for low-income seniors and individuals with physical disabilities.  

With a vision to rebalance the state’s long-term care system and reduce the reliance on institutional nursing home settings, Illinois created the SLP as a Medicaid-funded alternative that emphasizes independence, dignity, and quality care within a community-based environment.
